Overview


This article was developed to provide MeasurePM users with an overview of how to edit SOAP notes from Soap Note Settings, and how any changes made to the settings will impact submitted, saved and unsubmitted notes. 


Soap Note Settings


From Soap Note Settings, users are able to edit and make changes to SOAP notes by adding (i.e., mapping) properties, removing (i.e., unmapping) properties, activating properties to make them displayed in SOAP notes, and selecting which properties are visible to parents when they access SOAP notes via the Parent Portal. When changes are made, they only impact the SOAP notes associated to the selected Service Type.


Note that creating and editing SOAP notes require the applicable permissions to be granted. Below are all relevant permissions for creating and modifying SOAP notes.


To begin configuring SOAP notes, select 'Soap Note Settings' from the left hand navigation bar. From here, select the 'Service Type' for the SOAP note you would like to configure from the dropdown menu located at the top of the page. Once selected, all SOAP note system properties and created SOAP note custom properties will be displayed in the left side under 'Available Properties'. All properties listed here are available for mapping.


From the available properties list, locate the property you would like to map. Once selected, use the directional arrows between 'Available Properties' and 'Mapped Properties' to add the property to the SOAP note. Following mapping, you can make the optional checkbox selections. 


'Required' checkbox - These properties are mandatory and must be filled out in order to submit the SOAP note. All required sections will have an asterisk next to it on the SOAP note to indicate to users that they must complete it. Note that this checkbox is only be available for custom properties (explained below). 

'Active' checkbox - Only the properties with this checkbox selected will be visible in SOAP notes. Note that this checkbox is only be available for custom properties (explained below). 

'Parent Portal' checkbox - Only the properties with this checkbox selected are visible to parents when they access SOAP notes via the Parent Portal.
In order to save any change, users must select the blue 'Save' button in the bottom right hand corner of the screen. If Save is not selected all changes will be discarded. 


Property Types


There are two types of properties that can be mapped to SOAP notes.


1. Custom property - These properties are created by users in 'Soap Note Properties'. They are distinguishable from System properties in that they have the property type (Textbox, Checklist, Dropdown menu, etc.) shown in brackets directly to the right of the property name in the available properties list. 

2. System property - These properties are system generated properties that automatically pull relevant client and appointment information directly into the SOAP note (e.g., client name, Insurance ID, Provider NPI, etc.). Users cannot modify the 'Required' and 'Active' checkboxes for system properties, as they are always required and active once mapped to the SOAP note.


Below is a screenshot displaying both Custom and System properties. Remember, custom properties have the property type in brackets beside the name, system properties do not. 


Copy From


This helpful feature can be used to copy a SOAP note from a different Service Type to a new Service Type. Once copied, modifications can be made as needed. To use the Copy From feature, start by selecting a Service Type from the 'Service Type' dropdown. Next, select the 'Copy From' drop down menu and choose the service type for the note you would like to copy. After making a selection, all mapped properties and checkbox selections from the copied Service Type will appear in the Mapped Properties section.  


From here, the user can make additional modifications to the note if necessary. Note that the modifications will only be made for the SOAP note of the new service type, and it will not impact the note it was copied from. Modifications may include unmapping properties, mapping new properties found in the Available Properties window or modifying the checkbox selections.


Name

After mapping the all relevant properties to the new or copied note, the user can create a title for the SOAP note in the 'Name' textbox (e.g., Direct Session Note).


Save


After all changes have been made to the SOAP note, the user must select the blue 'Save' button. Without doing so, all changes will be discarded. 


Generate Preview


Once saved, the user can select 'Generate Preview' at the bottom of the screen as indicated by the arrow below. Once selected, a preview of the SOAP note will be displayed.



IMPORTANT: Making Changes to Previously Submitted SOAP Notes


Please be aware that changes made in Soap Note Settings WILL NOT impact any previously submitted SOAP notes. This is to ensure protection and maintenance of the information collected and formatting of already submitted SOAP notes.


Any changes made to the note in Soap Note Settings (i.e., mapping and unmapping properties and changing action items for properties) will be made for unsubmitted and saved as draft notes, all previously submitted notes will remain unchanged. For example, if a user make changes to Direct Behavior Therapy SOAP notes by mapping a new custom property, ONLY unsubmitted and saved as draft notes will have that specific property displayed. The newly mapped property will not be found in already submitted SOAP notes. However, as soon as the user resubmits a SOAP note, the change will be reflected. With this, should users want the most recent modifications to be displayed in already submitted SOAP notes, they must return the specific note of interest to draft if it was previously submitted, then either select 'Save as Draft' or 'Submit'.  


The steps to make changes to previously saved or submitted notes are as follows:


After making and saving modifications to the SOAP note settings for a specific Service Type, navigate to List View in My Appointments from the blue navigation bar on the left.


Next, make the applicable filter selections to generate a list of appointments or simply select the 'Filter' button to generate a list of all appointments in your site.


To locate the appointment and SOAP note of interest, users can use the 'Day', 'Week', and 'Month' buttons on the right, which modify how the appointments are displayed. The left and right arrows on the left side of the screen can also be used to change the day, week or month of appointments to view.


SOAP notes which have been submitted are indicated by a green icon in the SNotes column, notes which have been started and saved as draft are indicated as a red icon, and SOAP notes that have not yet been started are indicated by a grey icon. If changes to the note settings are made, green icon SOAP Notes will not have the most recent configurations, only the grey and red status notes will have been updated. 


Once a note is selected, the SOAP note will appear in a separate pop-up screen. To return to draft and resubmit, scroll down to the bottom of the window. Users must first select 'Return to Draft' then either 'Save as Draft' or 'Submit'. Please note that ability to return a submitted note to a draft is permission based, and therefore not all users will have availability to do so. As soon as it is returned to a draft and either saved as a draft or resubmitted, the most recent configurations will be visible.



To confirm that all changes made in Soap Note Settings are reflected in the resubmitted SOAP note, simply select the same SNote icon again and you will see all those relevant updates.


The same principal applies when saving and submitting SOAP Notes from the Mobile-App. All submitted will not be updated, but if you would like most recent changes to be reflected, simply return the submitted note to draft and either save as draft or submit it. 


Reminders:

  • Changes made to SOAP notes for a specific Service Type from the Soap Note Settings page WILL automatically be reflected in saved as draft and unsubmitted SOAP notes.
  • Changes made to SOAP notes for a Specific Service Type from the Soap Note Settings page WILL NOT be reflected in previously submitted SOAP notes.
  • If users want recent modifications to be visible in a previously submitted SOAP note, they must return the note to draft status and save the note as a draft or resubmit the specific SOAP note. This can be done from List View in Appointments on the Web or on the Mobile-App from SOAP Note History. 
  • The above points apply to the following changes: mapping and unmapping of System and Custom Properties, selecting or unselected the 'Active' checkbox and selecting or unselecting the 'Parent Portal' checkbox.
